FREE Covid-19 Test Kits are available to order for each household

Now, each residential household can order free at-home COVID-19 test kits by visiting covidtests.gov. Once ordered, the tests will usually ship in 7-12 days via USPS. Be sure to enter in an email address to receive shipping notifications.

Do you need a COVID-19 test now?

LGHIP members can still receive free tests at designated testing sites. Some independent pharmacies, Walgreens, CVS, Rite Aid, Walmart (in partnership with Quest Diagnostics), health centers and local health departments are providing COVID-19 tests. However, not all the above listed retailers have COVID-19 testing and many are by appointment only.

Some COVID-19 at home test kits are available for free.

Eligible test kits may be obtained for $0 by using your OptumRx card at the pharmacy counter at Walmart, Sam’s Club, Walgreens or Rite-Aid retailers. Covered LGHIP members can receive up to 8 individual tests per month. For example, a family of four would be eligible for 32 tests a month. For the test kit to be free, it must be an eligible at home test (see below) and submitted to the plan for coverage.

Which COVID-19 tests are eligible?

Coverage includes other the counter at-home COVID-19 test kits authorized by the Food and Drug Administration (FDA). The most common tests are BinaxNOW, Flowflex, InteliSwab, On/Go and QuickVue.

Have you purchased a COVID-19 test from a retailer since January 15?

You can request reimbursement for eligible test kits from OptumRx, our prescription drug administrator. Members can receive up to $12 per test reimbursed. To file for reimbursement, you may submit your claim by visiting optumrx.com/testinfo. A claim form can be submitted either electronically or by mail, but it will be processed faster if you submit an electronic claim. You will need to keep your purchase receipt(s) to submit for reimbursement. Complete this form if you need to submit it through the mail.
