Wellness Screenings
The LGHIP offers free wellness screenings to help you stay on top of your number one priority: your health. We’ll come to your workplace or you can visit one of many pharmacies in Alabama, your physician, or a public health department to complete your annual wellness screening. With some quick and easy testing, you know where you stand with your cholesterol, blood pressure, glucose, and more. The wellness screening program is available to all participants and non-Medicare retirees who are enrolled in the Plan (Group 30000), along with their covered spouses. Wellness screenings performed at the workplace, pharmacy or public health department are free. Screenings may also be performed by a primary care physician or nurse practitioner, however, applicable copays will apply. Your provider will need to complete this form if they conduct your wellness screening. Click here to view a list of the participating wellness pharmacy locations.
