2024 Pharmacy Benefits and Reimbursement Requests with OptumRX
The deadline to submit the Direct Member Reimbursement (DMR) by mail to OptumRX for eligible 2024 tier 2 and 3 prescription drugs is February 28, 2025.
OptumRx is the previous pharmacy benefit manager for members of the Local Government Health Insurance Plan. If you have questions about your OptumRx prescription drug reimbursements for 2024, please call 1-844-785-1603.
For name-brand prescriptions, you will need to pay 100% out-of-pocket at the point-of-sale, then file for your 80% reimbursement (subject to your $200 deductible) with OptumRx. Reimbursement requests may only be submitted to OptumRx by mail. We recommend maintaining a copy of the submitted form and receipts.
Note: Tier 1 and Tier 4 medications are not eligible for claim reimbursement.
